Corewell Health Urgent Care provides same-day camp and sports physical exams at several state-of-the-art urgent care clinics throughout Southeast Michigan, ensuring athletes and campers of all ages are healthy and cleared to play their favorite sports or head off to seasonal camps. Our certified medical providers make getting a physical exam for sports or camp fast and hassle-free.

We proudly offer physical exams for:

• Student-athletes (middle school, high school, and college)
• Club sports and recreational league players
• Campers attending physically active programs
• Anyone engaging in organized physical activity

All of our urgent care clinics are open from 8 a.m. to 8 p.m., seven days a week, providing access to quality that fits your schedule. Walk in or book ahead online to complete your physical exam so you can get out there and play!

A physical evaluates athletes' and campers’ health and wellness to ensure they're ready to play their sport or attend an active camp. The thorough examinations we provide will help identify any medical concerns that could impact performance or safety.

A sports physical exam includes the following:

• Medical History Review: Our medical providers will discuss past and present health issues, such as illnesses, injuries, and conditions like asthma or prior concussions, to ensure safe participation in sports.
• Vitals Check: Height, weight, blood pressure, and heart rate are measured to assess overall health.
• Vision Screening: A quick eye test confirms that athletes meet the vision requirements for their sport.
• Joint & Flexibility Assessment: To identify potential injury risks, we check joint stability, mobility, and flexibility are checked to identify potential injury risks.
• General Fitness Evaluation: The ears, nose, throat, heart, lungs, and abdomen are examined to assess cardiovascular and respiratory health and determine if any activity limitations are necessary.

Required Forms and Documentation

• Preparticipation Physical Evaluation forms must be completed in full before registration.
  • Please fill out the form for the current athletic season
• Your or your child’s glasses or contact lenses if they are needed to complete the vision test.
• Photo ID (if required by your school or organization).

Medical Information

• A list of current medications and any known allergies.
• Details on past injuries, surgeries, or chronic conditions (such as asthma or seasonal allergies).

Parent or Guardian Requirement

• Minors (under the age of 18) must be accompanied by a parent or legal guardian to their visit.
  • If a minor arrives without a parent or legal guardian, we will not be able to complete the exam.

*Please note that additional lab tests or services beyond the standard physical may incur extra charges. We do not provide certificates of immunization; these should be obtained from the original administering provider.
