What Should I Bring to a Physical Exam for Sports or Camp?
To have an efficient visit, please bring the following to a sports physical appointment for yourself or your child:
Required Forms and Documentation
- Preparticipation Physical Evaluation forms must be completed in full before registration.
- Please fill out the form for the current athletic season
- Your or your child’s glasses or contact lenses if they are needed to complete the vision test.
- Photo ID (if required by your school or organization).
Medical Information
- A list of current medications and any known allergies.
- Details on past injuries, surgeries, or chronic conditions (such as asthma or seasonal allergies).
Parent or Guardian Requirement
- Minors (under the age of 18) must be accompanied by a parent or legal guardian to their visit.
- If a minor arrives without a parent or legal guardian, we will not be able to complete the exam.
*Please note that additional lab tests or services beyond the standard physical may incur extra charges. We do not provide certificates of immunization; these should be obtained from the original administering provider.
